Several tests may be recommended to diagnose an enlarged prostate (BPH), depending on your symptoms and overall health. These commonly include a Digital Rectal Examination (DRE), PSA blood test, urine test, ultrasound, uroflowmetry, and post-void residual (PVR) urine measurement. These tests help confirm BPH, assess its severity, and rule out other urinary conditions.

If you have symptoms such as frequent urination, weak urine flow, difficulty emptying your bladder, or waking up multiple times at night to urinate, your urologist may recommend one or more tests to determine the cause.

The goal is to accurately diagnose BPH while excluding other conditions such as urinary tract infections, bladder problems, urethral strictures, or prostate cancer.

1. Digital Rectal Examination (DRE)

A Digital Rectal Examination (DRE) is a quick physical examination in which the doctor gently examines the prostate through the rectum to assess its size, shape, and consistency.

2. PSA Blood Test

The Prostate-Specific Antigen (PSA) blood test measures the level of PSA in your blood. Elevated PSA levels may be associated with BPH, prostatitis, or prostate cancer, so the result is interpreted together with your symptoms and examination findings.

3. Urine Test

A urine test helps identify:

  • Urinary tract infections (UTIs)
  • Blood in the urine
  • Other urinary abnormalities that may cause symptoms similar to BPH

4. Ultrasound

Ultrasound imaging may be used to evaluate:

  • The size of the prostate gland
  • The bladder
  • The kidneys (when required)
  • The amount of urine remaining in the bladder after urination

5. Uroflowmetry

Uroflowmetry measures the speed and strength of your urine flow. A slow or weak urine stream may indicate obstruction caused by an enlarged prostate.

6. Post-Void Residual (PVR) Measurement

A PVR test measures the amount of urine left in the bladder after urination. A high residual urine volume may suggest that the enlarged prostate is preventing complete bladder emptying.

Are all these tests necessary?

Not always. Your urologist will recommend only the tests that are appropriate based on your age, symptoms, medical history, and physical examination. Some patients may require only a few tests, while others with more severe symptoms may need a more detailed evaluation.
