ESWL (Extracorporeal Shock Wave Lithotripsy) is a non-invasive treatment that uses focused shock waves to break kidney stones into tiny fragments, allowing them to pass naturally through the urine. It is commonly recommended for selected small to medium-sized kidney stones and does not require any surgical incision.

Extracorporeal Shock Wave Lithotripsy (ESWL) is one of the least invasive treatments available for kidney stones. Unlike surgery, ESWL does not involve any cuts or instruments entering the urinary tract. Instead, high-energy shock waves are directed from outside the body to break the stone into small pieces. 

Once fragmented, these pieces usually pass naturally through the urine over the following days or weeks. 

How is ESWL performed?

The procedure generally involves the following steps:

  • The patient lies on a specially designed treatment table. 
  • Imaging such as ultrasound or X-ray is used to accurately locate the kidney stone. 
  • Focused shock waves are directed toward the stone. 
  • The stone gradually breaks into smaller fragments. 
  • The tiny fragments pass naturally through the urinary tract over time. 

The procedure is usually completed within 30–60 minutes, and many patients can return home the same day.

Who is a good candidate for ESWL?

ESWL may be suitable for patients who have:

  • Small to medium-sized kidney stones. 
  • Stones located in an appropriate position for shock wave treatment. 
  • No significant urinary blockage or active infection. 
  • Good kidney function. 

However, ESWL may not be the best option for very large stones, hard stones, obese patients, or certain stone locations. In such cases, procedures like RIRS, URS, or PCNL may provide better results.
