A hydrocele is a collection of fluid around the testicle, while epididymitis is inflammation of the epididymis, the tube located behind the testicle. Epididymitis commonly causes pain and tenderness, whereas a hydrocele is usually painless.

Although both conditions can cause scrotal swelling, they affect different structures and require different treatment.

Hydrocele:

  • Involves fluid collecting around the testicle
  • Usually causes painless swelling
  • May produce heaviness or pressure when large
  • May be monitored if small and asymptomatic
  • Persistent symptomatic cases may require hydrocele treatment or surgery

Epididymitis:

  • Involves inflammation of the epididymis
  • Often causes pain and tenderness in the scrotum
  • May be accompanied by redness, warmth, urinary symptoms, or fever
  • Can sometimes cause a reactive hydrocele
  • Treatment depends on the underlying cause and may include appropriate medication and supportive care

Because epididymitis can sometimes occur together with a hydrocele, persistent or painful scrotal swelling should be evaluated rather than diagnosed based on symptoms alone.
