Is a hydrocele dangerous?

A hydrocele is usually not dangerous and often does not cause serious complications. However, persistent, rapidly increasing, painful, or unexplained scrotal swelling should be evaluated to confirm that it is actually a hydrocele and not another condition.

A hydrocele is a collection of fluid around the testicle. In many adult men, it is a benign condition that mainly causes scrotal swelling, heaviness, or discomfort.

A hydrocele may become troublesome when it:

• Becomes significantly enlarged
• Causes persistent discomfort or heaviness
• Interferes with walking, exercise, or daily activities
• Develops after an infection, injury, or another scrotal condition
• Is associated with symptoms suggesting another underlying problem

A hydrocele itself generally does not damage the testicle. However, not every scrotal swelling is a hydrocele. Conditions such as an inguinal hernia, infection, testicular tumour, or testicular torsion can also cause swelling or discomfort and require different management.

For this reason, a new or unexplained scrotal swelling should be examined by a urologist. A physical examination and, when needed, scrotal ultrasound can help establish the diagnosis.

Treatment is not always immediately necessary when a hydrocele is small and symptom-free. A persistent or troublesome hydrocele may require surgical treatment such as hydrocelectomy.
