Can Hydrocele Come Back After Surgery?

Yes, hydrocele can recur after surgery, although recurrence is uncommon after definitive hydrocelectomy. The risk depends on the surgical technique, type of hydrocele, and individual healing.

Hydrocelectomy is intended to provide a long-term solution by treating the hydrocele sac rather than simply removing the accumulated fluid. However, no surgical procedure can completely eliminate the possibility of recurrence.

After surgery, some swelling or fluid around the testicle can occur as part of normal healing and does not necessarily mean that the hydrocele has returned.

Recurrence should be considered if:

• Scrotal swelling continues to increase after the initial recovery period
• A new fluid collection develops after the swelling had substantially settled
• The scrotum gradually becomes enlarged again
• Persistent or increasing heaviness develops

If swelling returns, a urologist can examine the area and use scrotal ultrasound when needed to determine whether it represents recurrent hydrocele, postoperative fluid, or another condition.
