A hydrocele is usually painless, but a large hydrocele can cause heaviness, pressure, or discomfort in the scrotum. Significant or persistent testicular pain should be evaluated because it may indicate another condition rather than the hydrocele itself.

A hydrocele is a collection of fluid around the testicle. In many men, the main symptom is painless scrotal swelling rather than pain.

When a hydrocele becomes large, it may cause:

  • A feeling of heaviness in the scrotum
  • Pressure around the testicle
  • Mild aching or discomfort
  • Discomfort during walking, exercise, or prolonged physical activity
  • A dragging sensation caused by the increased scrotal weight

However, severe, sudden, or persistent testicular pain should not automatically be attributed to a hydrocele. Conditions such as testicular torsion, epididymitis, orchitis, trauma, or an inguinal hernia can also cause scrotal pain and may require specific treatment.

A physical examination and, when necessary, scrotal ultrasound can help determine whether the hydrocele is responsible for the symptoms or whether another condition is present.
