Yes. A large hydrocele can cause heaviness, pressure, discomfort, and difficulty with activities such as walking, exercising, or sitting for long periods. Treatment may be considered when the swelling begins to interfere with daily life.

A small hydrocele may cause little more than visible scrotal swelling. As it becomes larger, however, the additional fluid and weight can become noticeable during everyday activities.

A large hydrocele may cause:

  • A feeling of heaviness or fullness in the scrotum
  • Pressure or discomfort while walking
  • Difficulty with exercise or physical activity
  • Discomfort while sitting for prolonged periods
  • Difficulty wearing certain clothing
  • Increasing concern about the appearance or size of the scrotum

A large hydrocele can also make examination of the underlying testicle more difficult. For this reason, a urologist may recommend an ultrasound when clinically appropriate.

Treatment depends on the severity of symptoms, size, progression, and underlying cause. Persistent, troublesome hydroceles may be evaluated for definitive treatment such as hydrocelectomy.
