Erectile dysfunction (ED) is the persistent inability to get or maintain an erection firm enough for satisfactory sexual activity. It is a common male sexual health condition and can be associated with blood vessel problems, diabetes, hormonal or neurological conditions, certain medicines, psychological factors, lifestyle factors, or other health conditions.

An erection normally depends on a coordinated interaction between the brain, nerves, hormones, blood vessels, and penile tissues. When one or more of these systems are affected, a man may have difficulty achieving or maintaining an erection.

ED can present in different ways, including:

  • Difficulty getting an erection.
  • Difficulty maintaining an erection during sexual activity.
  • Erections that are not firm enough for satisfactory intercourse.
  • Erections that occur inconsistently.
  • A gradual decline in erectile quality over time.

ED can have many possible causes. Common contributing factors include:

  • Diabetes and metabolic problems
  • High blood pressure and cardiovascular disease
  • Obesity and physical inactivity
  • Smoking and excessive alcohol use
  • Neurological conditions
  • Hormonal abnormalities, including low testosterone in some men
  • Certain medications
  • Anxiety, depression, stress, or relationship difficulties
  • Previous pelvic or prostate surgery
  • Penile conditions such as Peyronie's disease

ED is not simply an unavoidable part of ageing. Although erectile difficulties become more common with increasing age, persistent ED can sometimes be an early indicator of an underlying health problem, particularly vascular or metabolic disease.

The evaluation of ED therefore involves more than simply prescribing an erection medicine. A doctor may assess medical history, sexual history, medications, lifestyle factors, cardiovascular risk factors, and possible hormonal or other medical conditions.

Effective treatment is available for many men. Depending on the underlying cause and individual circumstances, treatment can include lifestyle changes, medicines, psychological or relationship support, vacuum erection devices, injections, or penile prosthesis surgery. The appropriate treatment depends on the diagnosis and patient preference.
