This is a very practical and common question. After a total hysterectomy, the cervix (the entrance to the uterus) has been removed. The surgeon then creates a "vaginal cuff" by stitching the top of the vagina closed. This means the vaginal canal is now a closed passage.
During intercourse, sperm is ejaculated into the vagina as it was before. However, since there is no longer a path to the uterus, the sperm simply stays in the vagina. It is then naturally broken down and absorbed by the body or leaves the body along with normal vaginal fluids. This is a completely harmless and natural process.
